Ivy Rehab Physical Therapy opens Second Location in Grand Rapids led by Dr. Zachary Eddy
Ivy Rehab Physical Therapy is now open in the Creston neighborhood of Grand Rapids, Michigan. The new clinic is located at 1264 Plainfield Avenue in Kent County. This team of dedicated experts welcomes new patients, providing flexible scheduling options to accommodate busy lifestyles and accepting most insurance plans for convenient access to care. Ivy Rehab’s vast network of over 500 locations, including nearly 40 in Michigan, makes high-quality care accessible close to home.
Zachary Eddy, PT, DPT, OCS, CSCS, serves as Clinic Director and Physical Therapist at the new Creston facility.

Zach is a compassionate, driven clinician who strives to deliver his patients the best possible, well-rounded care. He understands the vital importance of genuinely listening to people to create a collaborative rehab environment that treats the whole person.
Zach’s robust clinical background includes becoming a Board Certified Clinical Specialist in Orthopedics, Certified Strength and Conditioning Specialist, and certified in Dry Needling. He also has extensive training in Manual Therapy interventions, McKenzie, Mulligan, and Applied Functional Science techniques. He has been a mentor for an Orthopedics Residency Program and enjoys treating various conditions.
Zach has been in the West Michigan Area for the last six years and enjoys anything outdoors, including hiking, skiing, and spending time at the Lake. He is a big Michigan sports fan and enjoys spending time with his wife, son, and dog.
