Ivy Rehab Physical Therapy is now open in Andover, New Jersey. The clinic is conveniently located at 280 Newton Sparta Road behind Premier Health Orthopedics. The office is accepting new patients, accepts most insurance plans, and offers convenient hours. Chuck Smith, PT, MPT, serves as Clinic Director and Physical Therapist at the new Andover facility.

Chuck is a fun-loving, dedicated therapist with a wide range of expertise in many areas of physical therapy. He works to deliver individualized and the most up-to-date care to help his patients accomplish their goals. Chuck’s experience ranges from working with infants to the senior population. Chuck uses a blend of the latest technology and traditional hands-on care to produce outcomes that rank ahead of the reported national averages.
Chuck began as a physical therapist in 2000. He returned to direct patient care serving his home area in Sussex County in July of 2019 with Ivy Rehab Physical Therapy. He has been the Clinic Director at Ivy Rehab in Montague for the past two years.
Outside of work, Chuck enjoys spending time with his family and volunteering as a coach, referee, and organizer in youth sports and recreation.
