PATIENT INFORMATION
We at IvyRehab truly appreciate the time and energy that you are willing to put into your physical therapy program. As a partner in your care, we are committed to guiding and assisting you in all aspects of your rehabilitation program including; clinical questions, billing and financial inquiries, goal and long-term outcomes and after-care. Our clinical and office staff, as well as our management team, are always available to you if you have any questions about your care or our policies at any time during or following your time spent with IvyRehab.

Our therapists will work with you individually to assess and address your personal goals and return you to the highest level of function possible with the ultimate goal being the full return to your prior level of function before your injury or condition. While each case is unique, our goals are always to get you back to 100%. Specifically, your therapists will work with you to address the following as appropriate:
• Reduction/resolution of pain symptoms
• Return of mobility and range of motion
• Addressing strength limitations and training in an appropriate exercise program
• Balance and stability training
• Prevention of future injury
• Patient and family education
• Home exercise program and long term oversight of care
Your rehabilitation program will be overseen by your physician and supervised by a licensed physical therapist with assistance from ancillary and administrative staff. After a comprehensive initial evaluation which will include a personalized goal assessment and treatment plan, you will be scheduled for return visits based upon your physician’s prescription and your therapists recommendation. IvyRehab’s physical therapists will work with you and guide your therapy program with the primary objective of helping you achieve your goals and return you to your highest level of function as well as preventing further injury.
Please arrive 10-15 minutes prior to your evaluation in order to complete the appropriate paper-work. You can also complete our intake forms at your convenience by downloading forms from this page and bringing them to your first visit.
When you arrive at one of our facilities, our administrative staff will review your insurance coverage and your personal financial obligations based upon the verification of your benefits. Our staff will personally verify your benefits and explain to you all of the complexities and details of your insurance coverage. We will always be available to answer any of your questions or concerns regarding your benefits both prior to your first visit and as they may arise throughout the course of your care and even following your discharge from therapy.
You will then be evaluated by one of our licensed physical therapists. Your evaluation will be a one-on-one session in which your therapist will perform a thorough and comprehensive examination, noting any areas of deficiency or limitation in your function relating to your diagnosis or condition. Based upon the findings of your evaluation, you will be educated to your diagnosis and we will address each of your questions and concerns. You will also be issued a home exercise program that is individually prescribed by your clinician for you.
You should wear or bring comfortable clothes (including shoes and socks) that will allow access to your area of injury (i.e shorts for a knee/ankle), your physical therapy prescription, your insurance card and personal identification as well as your co-pay/deductible payment if applicable. Additionally, please be prepared to share your relevant past medical history and/or any medications you are on or allergies you may have with your treating therapist.
Yes, the physician/therapist relationship is a critical component of your therapy program. Our therapists will make every effort possible to regularly communicate your findings and progress to your referring physician. Following your evaluation and assessment, a copy of your initial evaluation note will be sent to your physician for their review and to go into your medical chart. Likewise, as your care continues, progress notes will be sent to update your doctor to relay your advancement towards your stated goals as well as your ongoing presentation. If at any time during your physical therapy treatment, your physician would like to relay his concerns or instructions, we welcome that communication and will directly integrate those suggestions into your care plan.
Please communicate your needs and/or questions to your clinician at any point. You may also use our Feedback Form to provide feedback. Your therapists and all of us at IvyRehab review all feedback from patients and use this feedback to both address immediate concerns and to continue to evolve the service we provide our patients, referral sources and the communities which we serve.
